Re: Domestic Partner Criteria
I am in the process of updating our Domestic Partner policy to meet the requirements of our mainline partner. As I make my way through the various travel databases; those who have domestic partners listed under their travel profiles may receive a request for additional documentation.
In order to include a domestic partner, the Affidavit of Domestic Partnership (form ET 1009) must be completed. Additionally employees need to provide two forms of supporting documentation that show financial inter-dependence such as documents showing a shared primary residence (joint mortgage, lease or deed records), joint banking account or credit cards, designation of the domestic partner as durable power of attorney, or other similar documentation of financial inter-dependence that is dated more than six-months to affirm the partnership.
